Conifer View AFH is a resident focused company that provides great services to senior population by offering customized care, independent or assisted living, therapy, and hospice in a safe home-like environment.

 

We strive to deliver health care with outstanding outcome, serve residents and their families with compassion, dignity, and integrity.

Conifer View AFH provides a family home setting for adults who are unable to live alone due to physical, emotional or developmental impairments. Our home provides 24 hour care for functionally impaired residents. Residents receive meals, support, supervision, and assistance with personal care and living skills as needed.

A higher level of service is being provided for the elderly which includes housekeeping, medication management, laundry, and also regular check-in on the residents. Basically, we are designed to bridge the gap between independent living and nursing home facilities.

Conifer View AFH facility provides Parkinson's and Alzheimer's care. This means it has special safeguards for wandering seniors, and the caregivers are trained in handling dementia behaviors.

Conifer View AFH provides hospice care which consists of home and facility care provided to benefit terminally ill patients and support their families through their tough times.
